 Dear United Health Care Patient:

 Effective April 1, 2011 Carolina Family Practice & Sports Medicine will no longer be able to bill your insurance company for lab work done in our office.   United Healthcare now requires all patients covered under their care to have lab work performed at a Lab Corp facility. 

 We realize this is an inconvenience for our patients.  It is for this reason that our partner in your care, Quest Diagnostics, has agreed to continue offering lab services to our United Healthcare patients who are out of network as a self-pay service.  If you choose to have your labs done in our office, you will receive an Explanation of Benefits from United Healthcare that reflects this change and you will be invoiced by Quest at a later time.  

You may choose to have your labs performed at an off-site Lab Corp facility where the service will be in-network and billed to your insurance company.  Our staff can provide you with a brochure that includes all of their Triangle locations as well as a Lab Corp requisition.  Please be advised this may result in a delay in getting your results from our office.  

 We apologize for any inconvenience this may have caused, but are satisfied that through our partnership with Quest Diagnostics, we are offering a viable solution.
