As CFPSM patients, you now have access to Duke MyChart, a secure portal to request appointments and communicate with your healthcare team.

To set up your MyChart Account, visit http://www.dukemychart.org and select “sign up now.”

When prompted for your activation code, please refer to the after visit summary you received at your last appointment.

If you have not had an office visit and wish to set up your MyChart account, please call 919-620-4555.

Forms

Below are common forms needed to transfer medical records to and from CFPSM.

Once forms are completed, please print the forms and send back to us using the following options:

By fax – Our fax number is #919-238-5010.

Mail them or drop them off at one of our three locations.

MyChart – If you have a MyChart login (our patient portal), you can send your provider a message with the form attached. If you do not have a login, please give us a call and we can email instructions to you to activate your account or you can call 1-800-782-6945.

M3132 Authorization to Release Protected Health Information_0 (this form allows CFPSM to send your medical records to another practice or facility)

MR Request for External Records General (this form allows you to have records from another provider or facility sent to CFPSM)

Verbal Release of Information Authorization Form (this form allows someone else to gain access to your medical history)

Insurance Information

Carolina Family Practice & Sports Medicine is an affiliate of Duke Health.  To view a list of accepted health insurance plans, please CLICK HERE.

Please check with your insurance carrier for any specific questions regarding your coverage (deductibles, preventative maximums or frequency, procedures, etc).  Our accepted insurances are subject to change, so please call to verify if we participate.

We are not responsible for referrals to our office for HMO plans.  If you have an HMO plan, it is your responsibility to select one of our providers as your PCP or obtain a referral from your primary care physician.

Please call our billing office with any further questions at 919-238-2000, and select option 5, then following the prompt, select option 2.  Please leave a detailed message as to how we can assist!!

Please note, we DO NOT participate with Worker’s Compensation or Auto Insurance (3rd party billing).

If you have an appointment with one of our providers, this is considered an office visit and a charge will be billed to your insurance carrier.

Patients with no insurance are eligible for a 50% discount.  (We cannot offer this discount if you have insurance.  We also cannot discount balances after they have been processed by your insurance).

Deductible – The amount you pay for your healthcare services before your health insurer pays.  Deductibles are based on your benefit period (typically a year at a time).  Example:  If your plan has a $2,000 annual deductible, you will be expected to pay the first $2,000 toward your healthcare services.  After you reach $2,000 your health insurer will cover the rest of the costs.

Coinsurance – A certain percent you must pay each benefit period after you have paid your deductible.  This payment is for covered services only.  You may still have to pay a copay.  Example: Your plan might cover 80 percent of your medical bill.  You will have to pay the other 20 percent.  The 20 percent is the coinsurance.

Travel Immunizations

Carolina Family Practice & Sports Medicine is pleased to provide comprehensive travel medicine to our patients who travel internationally.

Immunization and medication prescriptions are available on-site. The services are rendered based on CDC recommendations and individual health status and travel circumstances.

Available vaccines include:

  • Hepatitis A & B
  • Tetanus
  • Yellow fever (when available from the manufacturer)
  • Typhoid
  • Meningococcal

If you have questions about whether these vaccines are right for you, please consult the Center for Disease Control Website at http://www.cdc.gov, or ask your physician.

Laboratory Services

Carolina Family Practice & Sports Medicine is now partnering with LabCorp for our patients’ phlebotomy and laboratory needs.

An on-site lab can be found at our Cary, Raleigh and Holly Springs locations.

If your CFPSM provider has ordered laboratory tests that do not require an office visit with your provider, we ask that you please call to schedule a “Lab Only” visit.

While we realize this is new and an extra step on your part, it is necessary for us to place you on a schedule in order to ensure that your labs are not only ordered properly, but also ensures that the results come back in a timely manner for your provider to review them and communicate the results to you.

You can schedule your lab only visit by calling 919-238-2000, option 1.

Please also note that LabCorp facilities within our offices can only process labs ordered by a CFPSM provider and can NOT process self-pay labs.  To schedule an appointment for labs ordered by an outside provider or for self-pay labs, please visit www.labcorp.com to find a LabCorp patient service center near you.

Primary Care Medical Home (PCMH)

In January 2018, CFPSM began the journey to becoming a Primary Care Medical Home (PCMH). This is a certification awarded by the Joint Commission.

A primary care medical home is a model of healthcare centered around the needs of our patients.

As a primary care medical home, we partner with our patients to ensure all of their physical and mental healthcare needs are met. We do this by coordinating and managing all of the care they receive, including specialty care, community services and home healthcare services.
