This letter is to inform you that effective December 30 2021, I will be leaving Carolina Family Practice & Sports Medicine (CFPSM). This was a very difficult decision to make, as I have grown very fond of the patients and community of which we serve. I made a personal decision to further pursue my interests in sports medicine and regenerative medicine. It truly has been my privilege to care for you. I will miss all of you.
It is important for you to know that you have a choice of health care providers. The providers at CFPSM, an affiliation of Duke Private Diagnostic Clinic are committed to continuing to provide the highest quality care for you and your family.

Should you choose to seek care elsewhere, CFPSM will assist in the transition of medical records to your new provider. Please inquire at the front desk to authorize the release of your medical records. You may obtain your medical records by mailing the completed form to: Duke University Hospital, Health Information Management Department, Attn: Medical Information Release Unit, DUMC Box 3016, Durham, NC 27710. If you have any questions about the transfer of your medical records, contact the Duke University Hospital Health Information Release Unit at 919-384-7119.
