Carolina Family Practice & Sports Medicine Partnership with Novant Health
We are excited to announce that effective January 1, 2025, Carolina Family Practice & Sports Medicine (CFP&SM) will be entering into a new partnership with Novant Health. This decision follows extensive consideration and thorough due diligence by our partner physicians and management team. Learn more about Novant Health.
CFP&SM is dedicated to providing you information as it becomes available. Here is what we know:
- Our first priority is, and has always been, our patients.
- We believe this partnership will allow us to provide better and more comprehensive care to our patients, our communities and organizations we serve.
- All of our current CFP&SM providers will remain with our practice and we hope you choose to stay with us too.
- We remain committed to maintaining and advancing our status as a Patient-Centered Medical Home.
- You may receive a notification from your insurance company indicating that CFP&SM physicians are no longer in-network due to our departure from Duke Health. Please be assured that we will remain in-network with the majority of insurances.
- For additional information, please use the links below or you may also contact our office at 919-238-2000, option 5 (billing line).
- You medical records will continue to be accessible to your CFP&SM healthcare team. There will be no disruption in your care or in accessing your medical history.
- There will be no changes to your existing appointment schedule, including those scheduled in 2025. If there are any modifications, we will notify you in advance.
- We welcome you to speak with your provider as they can provide additional answers to potential questions and insight.

Patient Messaging Update 1/21/2025
You may have received confusing messages this month stating that if you had an upcoming appointment, you would need to click a link to reschedule, cancel or confirm your appointment. You also may have received an email stating that CFPSM was closed. These messages were sent by the healthcare system with which we were previously affiliated. These are automated messages that often cannot be proactively “turned off” and we apologize for any confusion these communications may have caused you.
As of 01.01.25, Carolina Family Practice & Sports Medicine is now part of Novant Health. CFPSM is on a completely different version of Epic that requires you, the patient, to activate a new Novant Health MyChart account and opt-in to a new Novant Health Hello World texting platform to communicate with us electronically. We encourage you to activate your new Novant Health MyChart account so you can see real-time the status of any upcoming appointments. You do not have to wait until you have an appointment scheduled to activate your Novant Health MyChart! Link your MyChart accounts to have all your records in one place.
If you’ve been seen at more than one healthcare organization, you likely have multiple health records. MyChart lets you pull all your medical records together and view them in one place. When you connect your accounts from participating healthcare organizations, information from those organizations appears right in your MyChart, including:
Allergies – Health History – Medications – Test Results
You will need to log into your Novant MyChart and connect your charts from other healthcare organizations. Instructions for how to make this connection from either the MyChart App or the Website are below.
1. Search “link” from your Menu.
2. Find the ‘Link with Other Organizations’ notification and click ‘Explore’.
3. You can then search for health organizations in the ‘Discover Accounts to Link’ search bar.
4. Once you select a health organization, you will be asked to select a verification question, such as your phone number or last four digits of your social security number to confirm the correct account is linked.
5. Once completed, your accounts will be linked, and you will be able to see information from other healthcare organizations when logged into your Novant MyChart. For more information, including an instructional video, you can click this link Happy Together | MyChart
