In late 2011, all CFPSM providers were recognized by the National Committee on Quality Assurance as Level III Patient Centered Medical Home. Visit the NCQA Recognition Directory (select Physician Practice Connections – Patient Centered Medical Home) to find your CFPSM provider listed, or to establish with a PatientCentered Medical Home at any of our three locations.
