You will need to switch to the Novant Health MyChart so you can stay connected to your care team and in control of your health journey. This is where you will be able to see your CFPSM records, test results and connect with your doctors — all from the convenience of any device.

See all your records in one place

If you’ve been seen at more than one healthcare organization, you likely have multiple health records. MyChart lets you pull all your medical records together and view them in one place. That puts all of your information right at your fingertips.

Connect your records in four simple steps

  1. Search “link” from your Menu.
  2. Find the ‘Link with Other Organizations’ notification and click ‘Explore’.
  3. You can then search for health organizations in the ‘Discover Accounts to Link’ search bar.
  4. Once you select a health organization, you will be asked to select a verification question, such as your phone number or last four digits of your social security number to confirm the correct account is linked.

Once completed, your accounts will be linked, and you will be able to see information from other healthcare organizations when logged into your Novant MyChart.

Telemedicine Now Offered

We are now offering telemedicine appointments.  Your provider will call you directly to assess your injury/illness.  We are utilizing this tool to help screen for COVID as well.

Insurance Information

Carolina Family Practice & Sports Medicine is an affiliate of Novant Health.  To view a list of health insurance plans and determine if we are in-network for you, CLICK HERE

Please check with your insurance carrier for any specific questions regarding your coverage (deductibles, preventative maximums or frequency, procedures, etc).  Our accepted insurances are subject to change, so please call to verify if we participate.

We are not responsible for referrals to our office for HMO plans.  If you have an HMO plan, it is your responsibility to select one of our providers as your PCP or obtain a referral from your primary care physician.

Please call our billing office with any further questions at 919-238-2000, and select the billing option.  Please leave a detailed message as to how we can assist.

Please note, we DO NOT participate with Worker’s Compensation or Auto Insurance (3rd party billing).

If you have an appointment with one of our providers, this is considered an office visit and a charge will be billed to your insurance carrier.

Patients with no insurance are eligible for a 50% discount if paid at the time of service.  (We cannot offer this discount if you have insurance.  We also cannot discount balances after they have been processed by your insurance).

Deductible – The amount you pay for your healthcare services before your health insurer pays.  Deductibles are based on your benefit period (typically a year at a time).  Example:  If your plan has a $2,000 annual deductible, you will be expected to pay the first $2,000 toward your healthcare services.  After you reach $2,000 your health insurer will cover the rest of the costs.

Coinsurance – A certain percent you must pay each benefit period after you have paid your deductible.  This payment is for covered services only.  You may still have to pay a copay.  Example: Your plan might cover 80 percent of your medical bill.  You will have to pay the other 20 percent.  The 20 percent is the coinsurance.

Pediatric Vaccination Schedule

At Carolina Family Practice & Sports Medicine, we are committed to the health and well-being of all our patients — especially children. We strongly support the use of vaccines as recommended by the Centers for Disease Control and Prevention (CDC) and required by the North Carolina Department of Health and Human Services (NCDHHS) for childcare and school attendance. Vaccines are a safe and essential part of preventive care and play a critical role in protecting children and the community from serious, preventable illnesses.

We understand that some parents or guardians may have questions or concerns about vaccines. We encourage open and respectful conversations, and we invite families to schedule a dedicated visit with their provider to discuss any vaccine-related questions. This gives us the opportunity to provide clear, evidence-based information and support informed decision-making.

Policy on Declined Vaccines

If a parent or guardian chooses to decline any vaccine that is required by the NCDHHS for childcare or school attendance, we require that an Against Medical Advice (AMA) form be signed. This form acknowledges that the decision goes against current medical and public health guidelines and ensures appropriate documentation in the child’s medical record.

Why This Policy Matters

Following recommended vaccine schedules not only protects your child but also helps safeguard children who cannot be vaccinated due to medical reasons. We appreciate your partnership in keeping all families in our community healthy and safe.

Travel Immunizations

Carolina Family Practice & Sports Medicine is pleased to provide comprehensive travel medicine to our patients who travel internationally.

Immunization and medication prescriptions are available on-site. The services are rendered based on CDC recommendations and individual health status and travel circumstances.

Available vaccines include:

  • Hepatitis A & B
  • Tetanus
  • Yellow fever (when available from the manufacturer)
  • Typhoid
  • Meningococcal

If you have questions about whether these vaccines are right for you, please consult the Center for Disease Control Website at http://www.cdc.gov, or ask your physician.

Laboratory Services

Carolina Family Practice & Sports Medicine is now partnering with LabCorp for our patients’ phlebotomy and laboratory needs.

An on-site lab can be found at our Cary, Raleigh and Holly Springs locations.

If your CFPSM provider has ordered laboratory tests that do not require an office visit with your provider, we ask that you please call to schedule a “Lab Only” visit.

While we realize this is new and an extra step on your part, it is necessary for us to place you on a schedule in order to ensure that your labs are not only ordered properly, but also ensures that the results come back in a timely manner for your provider to review them and communicate the results to you.

You can schedule your lab only visit by calling 919-238-2000, option 1.

Please also note that LabCorp facilities within our offices can only process labs ordered by a CFPSM provider and can NOT process self-pay labs.  To schedule an appointment for labs ordered by an outside provider or for self-pay labs, please visit www.labcorp.com to find a LabCorp patient service center near you.

In January 2018, CFPSM began the journey to becoming a Primary Care Medical Home (PCMH). This is a certification awarded by the Joint Commission.

A primary care medical home is a model of healthcare centered around the needs of our patients.

As a primary care medical home, we partner with our patients to ensure all of their physical and mental healthcare needs are met. We do this by coordinating and managing all of the care they receive, including specialty care, community services and home healthcare services.
