To set up your MyChart Account, visit http://www.dukemychart.org and select “sign up now.”
When prompted for your activation code, please refer to the after visit summary you received at your last appointment since June 22.
If you have not had an office visit since June 22 and wish to set up your MyChart account, please call 919-620-4555.

Carolina Family Practice & Sports Medicine is currently contracted with the following list if insurance companies (effective November 10, 2016). Please check with your insurance carrier for any specific questions regarding your coverage (deductibles, preventative maximums or frequency, procedures, etc). Our accepted insurances are subject to change, please call to verify if we participate.
We are not responsible for referrals to our office for HMO plans. If you have an HMO plan, it is your responsibility to select one of our providers as your PCP or obtain a referral from your primary care physician. Please see a member of our billing department with any questions.

- Blue Cross & Blue Shield (including Partner’s/HMO, Blue Medicare, and Anthem). We DO NOT participate with Blue Value (prefixes of YPV, YPJ, YPL, YPS).
- We will participate with a plan called BLUE LOCAL effective November 1, 2016. This plan can be purchased on the healthcare marketplace and is the ONLY plan from the healthcare marketplace that we will participate with. If choosing Blue Local, please specify that the plan is for DUKE / WAKEMED.
- Carolina Access – REFERRAL REQUIRED, however we cannot accommodate well-child care for Medicaid patients under the age of 18 because we do not carry state-funded vaccines. The only exception to this are sports-related concussions and musculo-skeletal injuries which still require a referral from the patient’s PCP.
- Cigna (We will NOT participate with Cigna Connect)

Please note, we DO NOT participate with Worker’s Compensation or Auto Insurance (3rd party billing).
If you have an appointment with one of our providers, this is considered an office visit and a charge will be billed to your insurance carrier. There are very few exceptions to this rule. Please contact the billing office with specific questions.
We no longer participate with Beechstreet.
Patients with no insurance are eligible for a 50% discount.

We’d also like to share a working definition of what a deductible is, and how it relates to insurance: “In an insurance policy, the deductible is the portion of any claim that is not covered by the insurance provider. It is the amount of expenses that must be paid out of pocket before an insurer will cover any expenses. The first commercial deductible was introduced by Norman Baglini in 1952. It is normally quoted as a fixed quantity and is a part of most policies covering losses to the policy holder. The deductible must be paid by the insured, before the benefits of the policy can apply. Prior to your child’s well visit, you may be asked to fill out an Age & Stages Questionnaire. This will help your provider understand your child’s development.
Carolina Family Practice & Sports Medicine is pleased to provide comprehensive travel medicine to our patients who travel internationally.
Immunization and medication prescriptions are available on-site. The services are rendered based on CDC recommendations and individual health status and travel circumstances.
Available vaccines include:
- Hepatitis A & B
- Yellow fever (when available from the manufacturer)
If you have questions about whether these vaccines are right for you, please consult the Center for Disease Control Website at http://www.cdc.gov, or ask your physician.
Carolina Family Practice & Sports Medicine is now partnering with LabCorp for our patients’ phlebotomy and laboratory needs.
An on-site lab can be found at our Cary location.
Within the American Institute of Healthcare & Fitness (home of our Raleigh location), LabCorp can be found in Suite 223.
In Holly Springs, a LabCorp Patient Service Center is located approximately two miles from our Rosewood Centre Drive office. Located at 104 Bass Lake Road, Suite 100, Holly Springs, NC 27540, this PSC’s hours are Monday-Friday, 8a-5p (closed 12p-1p).
If your provider has ordered laboratory tests that do not require an office visit with your provider, we ask that you please call to schedule a “Lab Only” visit.
While we realize this is new and an extra step on your part, it is necessary for us to place you on a schedule in order to ensure that your labs are not only ordered properly, but also ensures that the results come back in a timely manner for your provider to review them and communicate the results to you.
Please also note that only labs ordered by a CFPSM provider can be performed at the LabCorp facilities within our offices.
